Dr. Shauna Putzy, PsyD, LPC 

Clinical Director

Dr. Putzy is a Clinical Psychologist and has worked in a variety of settings; including managed care organizations, outpatient counseling, public assistance, and crisis intervention services.  She holds a master’s degree in mental health counseling and a doctorate degree in clinical psychology.  Dr. Putzy focuses on anxiety/depression, life transitions, grief and loss, perinatal anxiety, family planning, and relationship challenges. She utilizes a variety of techniques with an emphasis on Cognitive Behavioral Therapy (CBT) and Solution Focused Therapy.  She focuses on the overall health and wellness of the individual.

Dr. Putzy will be offering psychological testing beginning in June for adolescents and adults with concerns related to attention/concentration and clarifying psychological diagnosis.  Accurate diagnosis allows for successful medication management.

Jessica Modjeski, MSW, LCSW

Crystal Reines, LPC-IT

Crystal will be joining the team in May 2026.

Crystal Reines is a Licensed Professional Counselor in Training and holds a Master of Science in Clinical Mental Health Counseling. She has experience working with children, adolescents, and adults. Crystal supports clients navigating concerns such as depression, anxiety, post-traumatic stress disorder, ADHD, trauma, and relationship conflict. She is passionate about helping individuals build coping skills, improve communication, and work through life’s challenges in a supportive and nonjudgmental environment.

Crystal uses an integrative approach to therapy that includes person-centered therapy, cognitive behavioral therapy (CBT), trauma-focused CBT, solution-focused therapy, and dialectical behavior therapy (DBT). She believes therapy should be collaborative and tailored to each individual’s needs and is particularly interested in incorporating EMDR into her work in the future to further support clients processing trauma.

Outside of her professional work, Crystal enjoys traveling with her family and exploring new places whenever she has the opportunity.

Jordan Trent, MS, LPC

Jordan will be joining the team in May 2026.

Jordan Trent is a Licensed Professional Counselor with a Master of Science in Clinical Mental Health Counseling from the University of Wisconsin- Stout. Jordan primarily works with adults and views therapy as a space for people to share their stories, increase self-awareness, heal old wounds, and develop new ways of navigating the world. She believes that one of the bravest things a person can do is be vulnerable and deeply values the opportunity to witness that courage in her clients. She places a special emphasis on the therapeutic relationship, recognizing that connection is the basis of all change. Her approach is collaborative and flexible, meeting each client where they are and adapting to their unique needs.

Jordan utilizes a combination of Psychodynamic Therapy, Cognitive Behavioral Therapy (CBT), Acceptance and Commitment Therapy (ACT), and Relational Psychotherapy in her work. She is also trained in Eye Movement Desensitization and Reprocessing (EMDR). Jordan has a special interest in working with anxiety, trauma, grief, life transitions, and identity conflicts.

Outside of the office, Jordan enjoys traveling, trying new coffee shops, and spending time in nature- especially near the water.
